Requirements of Duplicate Category A or B (Damaged or Worn Out Cases)

 

 

1. Application for Duplicate Category with reasons

2. Two (2) colored photocopies of CNIC

3. Three (3) specimen signatures duly attested

4. Four (4) Photograph Passport size

5. Surrendering the damage  / worn out Registration Certificate in Punjab Pharmacy Council

6. Pay Order / Demand Draft for issuance of Duplicate Certificate Fee in the name of Secretary, Punjab Pharmacy Council of Rs. 5000/-.

7. Registration Copy if available

8. Original Registration File

9. Personal Appearance in Punjab Pharmacy Council / Appearance certificate from Area Drug Inspector                              (if necessary).
